Qualifications:

  • Active and unencumbered LPN license
  • Graduate from an accredited nursing program
  • Current BLS certification
  • Previous experience in a clinical or long-term care setting preferred
  • Excellent interpersonal and communication skills
  • Strong attention to detail and ability to follow care protocols
  • Ability to work collaboratively with physicians, RNs, and healthcare staff

Key Responsibilities:

  • Provide direct, day-to-day patient care and monitor patient health
  • Administer prescribed medications and document responses
  • Assist with patient assessments, vital signs, and routine procedures
  • Communicate changes in patient status to clinical team and families
  • Support care team with various clinical tasks as assigned
  • Maintain up-to-date, accurate records in compliance with facility policies
