Registered Nurse - Home Health in Boston, MA

Deliver compassionate care and support as a Home Health Registered Nurse, serving patients in the Boston area. This contract position centers on providing high-quality, patient-focused nursing in a home setting. As a critical member of a multidisciplinary team, you will assess conditions, implement skilled care plans, and offer vital education to patients and their families, all while supporting their recovery and independence at home. This role requires a strong foundation in community health practice and exceptional case management skills.

Schedule:

• Weekend rotations are required once every 6 weeks

Required Qualifications:

• Active Registered Nurse (RN) license
• Minimum 1 year of certified home care experience
• At least 1 year of case management experience
• Ability and willingness to travel to assigned patient cases
• Proficiency in electronic documentation
• Strong communication and critical thinking skills
• Commitment to delivering patient-centered care within organizational and community health standards

Key Responsibilities:

• Assess patient health status, develop and implement personalized care plans
• Deliver direct nursing care, monitoring patient progress and adjusting interventions as necessary
• Educate patients and families on disease processes, treatments, and self-care strategies
• Perform admission assessments and discharge planning in compliance with organizational requirements
• Participate in interdisciplinary collaboration and consistent communication
• Maintain accurate, timely, and thorough documentation in the electronic medical record
• Adhere to organizational philosophy, policies, and professional standards of community health practice
