(RN) Registered Nurse - Home Health - Boston, Massachusetts

Seeking a skilled Registered Nurse to deliver high-quality home health services to patients across the lifespan. This contract opportunity involves independent patient visits throughout the greater Boston area, supporting individuals and families directly in their homes.

Schedule/Hours:

• Participation in weekend rotation (every 6 weeks)
• Willingness to work holidays and participate in on-call coverage according to department needs

Qualifications:

• Current Massachusetts RN licensure required
• Minimum of 1 year certified home care nursing experience
• At least 1 year of direct case management experience
• Reliable personal transportation for travel to patient residences
• Flexible and committed to working weekends, holidays, and on-call as scheduled

Responsibilities:

• Provide comprehensive in-home clinical services to patients ranging from newborns to the elderly
• Conduct patient assessments, develop and implement individualized care plans
• Deliver patient and family education, promote health, and support ongoing care needs
• Manage the admissions process and coordinate case management activities
• Adhere to organizational standards, policies, and best practices for home health care delivery
• Utilize electronic documentation efficiently and communicate effectively with the interdisciplinary team to ensure continuity and quality of care
