This contract nursing role focuses on supporting individuals from infancy to senior age within the comfort of their own homes, ensuring they receive the quality care they need and deserve.
Required Qualifications:
- Active RN license in Massachusetts
- Minimum 1 year of certified home care experience
- At least 1 year of case management background
- Valid, independent means of reliable transportation to perform patient home visits
- Willingness to work weekends, holidays, and participate in on-call rotation (weekend coverage is approximately once every six weeks)
Responsibilities:
- Deliver comprehensive clinical services to a diverse age group, from newborns to centenarians, within home settings
- Conduct thorough patient assessments and implement individualized care plans in collaboration with the interdisciplinary team
- Provide ongoing education and support to patients and their families to promote patient well-being and health literacy
- Manage a caseload that includes timely admissions, follow-ups, and case management compliance according to departmental policies
- Maintain accurate and prompt electronic documentation and coordinate care through effective communication with colleagues and care teams
- Adhere to established standards of practice to ensure the highest quality of patient care
