Make a meaningful difference in patients’ lives by providing high-quality care in a home-based setting as a Registered Nurse specializing in Home Health. This contract opportunity calls for a compassionate and organized RN with strong clinical knowledge and a commitment to community health.
Qualifications & Experience:
- Active RN license in good standing
- Minimum of 1 year of certified home care nursing experience
- At least 1 year of case management experience
- Proficient with electronic documentation and effective interdisciplinary communication
- Ability and willingness to travel to patient homes as assigned
- Comfortable working independently and adapting care to individual needs
Schedule:
- Weekend rotation required once every 6 weeks
Key Responsibilities:
- Conduct thorough patient assessments to determine care needs and develop individualized care plans
- Implement, monitor, and adjust plans of care to ensure the best possible outcomes
- Perform patient admissions and discharge processes, maintaining compliance with organizational policies and standards
- Educate patients and their families on ongoing care, disease management, and community resources
- Collaborate effectively with physicians, therapists, and other members of the care team to ensure coordinated service delivery
- Maintain detailed and timely electronic health records reflecting each patient’s progress and interventions
