RN Home Health - Brockton, Massachusetts

Provide vital support and compassionate care to patients in the comfort of their own homes as an experienced Home Health Registered Nurse. In this contract role, you will collaborate with a dedicated interdisciplinary team, delivering high-quality nursing care while exercising your critical assessment skills and case management expertise.

Schedule/Hours:

• Weekend rotations are required once every 6 weeks

Qualifications & Experience:

• Active RN license
• Minimum 1 year of certified home care nursing experience
• At least 1 year of case management experience
• Proven ability to travel to assigned patient cases within the service area
• Proficiency in electronic health documentation and interdisciplinary communication
• Strong clinical assessment, organizational, and patient education skills

Key Responsibilities:

• Conduct comprehensive assessments of patient conditions in their home environments
• Develop and implement individualized care plans in alignment with established standards and policies for community health nursing
• Provide thorough patient and family education to support the care process and promote optimal health outcomes
• Perform admission processes and ongoing case management, including collaboration and timely communication with the care team
• Ensure all patient care is documented clearly and promptly using electronic health records
• Maintain professional standards of practice and ensure compliance with organizational policies
