Home Health RN in Brockton, MA

Schedule/Hours:

• Weekends/holiday/on-call coverage based on department rotation
• Weekend rotations required once every 6 weeks

Qualifications:

• Active RN license in Massachusetts
• At least 1 year of certified home care experience
• Minimum 1 year of case management experience
• Commitment to home visits with dependable, independent transportation
• Willingness to provide care during weekends, holidays, and on-call shifts as part of regular rotation

Key Responsibilities:

• Deliver direct clinical services to patients ranging from infants to seniors, adapting care to diverse needs
• Assess patient status, develop individualized care plans, and provide evidence-based interventions
• Educate patients and families, empowering them with tools to support recovery and manage conditions at home
• Conduct home admissions and ongoing case management, adhering to professional practice standards and internal guidelines
• Maintain thorough, timely electronic documentation and foster interdisciplinary communication to ensure cohesive care
