(RN) Registered Nurse - Home Health - Brockton, Massachusetts

Schedule/Hours:

• Weekend coverage is required once every 6 weeks
• Must be open to weekend, holiday, and on-call shifts according to department rotation

Qualifications:

• Active RN license in the state of Massachusetts
• Minimum of 1 year of certified home care experience
• At least 1 year of case management background
• Reliable vehicle and the ability to independently travel to patient homes
• Dedicated to flexible scheduling, including weekends, holidays, and shared on-call responsibilities

Responsibilities:

• Deliver comprehensive clinical services to individuals from birth through late adulthood in their homes
• Perform thorough patient assessments, develop and implement care plans, and adjust care as patient needs evolve
• Provide education and ongoing support to patients and their families to promote self-sufficiency and optimal health outcomes
• Lead or participate in the admission process for new patients, ensuring detailed and timely electronic documentation
• Uphold policies, standards of practice, and interdisciplinary communication protocols to optimize patient outcomes and satisfaction
