Registered Nurse - Home Health - Cambridge, MA

Schedule/Hours:

• Weekend rotations required, approximately once every 6 weeks
• Participation in weekend, holiday, and on-call coverage according to department needs

Qualifications:

• Current RN licensure in the state of Massachusetts
• Minimum of 1 year of certified home care experience
• At least 1 year of case management experience
• Reliable, independent transportation to travel between patient homes
• Flexibility and willingness to participate in weekend, holiday, and on-call rotations

Responsibilities:

• Provide comprehensive clinical nursing services to patients of all ages in their homes
• Assess patient status, develop individualized care plans, and adapt care as needed
• Educate and support patients and families regarding treatment plans, medication management, and self-care strategies
• Complete admissions processes and maintain accurate, timely electronic documentation
• Communicate effectively with interdisciplinary healthcare team members to ensure continuity and quality of care
• Comply with all applicable department policies and standards of practice
