Hospice (RN) Registered Nurse - Home Health | Edison, New Jersey

Advance your nursing career by providing compassionate hospice and home health care to patients and families during sensitive times. This contract role offers the opportunity to make a lasting impact, working closely with an interdisciplinary team in a dynamic homecare setting.

Schedule:

• 37.5 hours per week
• 7.5-hour shifts, 8:00 am – 4:00 pm
• Five days per week with every other weekend required (comp days provided)
• Floating to different counties as needed
• Expect 4–5 daily visits, including admissions

Qualifications:

• Current New Jersey RN license
• Associate’s degree in Nursing (ASN) or Nursing diploma
• Minimum two years of RN experience in home hospice care
• Valid New Jersey driver’s license and BLS certification
• Proficient English reading, writing, and communication skills
• Experience with EMR systems and strong critical thinking abilities
• Excellent patient assessment, care planning, case management, and interdisciplinary communication skills

Responsibilities:

• Conduct thorough patient assessments and determine hospice eligibility
• Develop, evaluate, and revise individualized care plans for patients and families
• Provide hands-on, evidence-based nursing care in accordance with hospice regulations
• Utilize age-appropriate education methods; document patient teaching tools
• Complete timely admission, discharge, and revisit documentation using EMR
• Maintain productive visit schedules and comply with insurance guidelines
• Reconcile medications and uphold safe medication management practices
• Attend bi-weekly interdisciplinary team meetings and contribute to case reviews
• Participate in staff meetings, orientation, mentorship, and agency growth initiatives
• Uphold infection control measures and promote a safe environment

$2,001 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 36 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
