Hospice (RN) Registered Nurse - Home Health | Elizabeth, New Jersey

Deliver compassionate, patient-centered care as a Registered Nurse specializing in home hospice services. This contract opportunity focuses on supporting individuals and families during critical times, emphasizing holistic case management and interdisciplinary collaboration.

Schedule:

• Hours per week: 37.5
• Daily shift: 8:00am–4:00pm
• Days per week: 5, including every other weekend (with compensatory days during the week)
• Expect 4–5 patient visits daily, plus admissions
• Flexibility to float across counties as needed

Qualifications:

• Current New Jersey RN license (required)
• Associate’s degree in nursing, or diploma in nursing
• At least 2 years’ experience as an RN in home hospice (required); 3+ years preferred
• Valid New Jersey driver’s license
• Basic Life Support (BLS) certification
• Proficiency with EMR systems, strong critical thinking and communication skills
• English fluency (speak, read, and write)

Responsibilities:

• Perform thorough patient assessments to determine hospice eligibility and care needs
• Develop, implement, and regularly evaluate individualized care plans in collaboration with patients and families
• Complete patient admissions, discharges, and daily documentation in a timely manner
• Educate patients and families using age-appropriate teaching methods and document materials used
• Manage all aspects of patient case management, prioritizing care and ensuring regulatory compliance
• Coordinate closely with the interdisciplinary team, participate in IDT meetings biweekly, and communicate patient progress
• Adhere to infection control guidelines and safe medication practices, including routine medication reconciliation
• Play an active role in performance improvement initiatives and staff education

$2,001 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 36 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
