RN Home Health - Ewa Beach, Hawaii

Provide expert nursing care as an integral member of a home health team committed to patient-centered outcomes in Ewa Beach, HI. This contract position offers an opportunity to use your clinical expertise to influence care coordination and case management for diverse and complex patient populations.

Key Responsibilities:

• Collaborate with patients, families, physicians, nurses, and multidisciplinary teams to implement and revise individualized care plans.
• Act as a liaison ensuring effective communication and continuity between healthcare providers and beneficiaries.
• Lead coordination efforts among care disciplines, monitoring timeliness and appropriateness of treatments, procedures, and diagnostic testing.
• Consult with physicians regarding resource utilization, treatment plans, length of stay, and discharge preparation.
• Manage care for complex patients including frequent emergency department visitors, chronic pain sufferers, substance abuse patients, and individuals experiencing homelessness.
• Work closely with acute care case managers to ensure seamless transitions and ongoing care appropriateness.
• Oversee patient transfers to tertiary care centers ensuring continuity of care with proper documentation.
• Collect and manage data to support regulatory and accreditation compliance.
• Conduct focused study reviews on frequent emergency department utilizers to improve patient outcomes.

Desired Experience:

• Current Registered Nurse (RN) license.
• Proven background in home health nursing and case management.
• Experience managing complex patient cases with a multidisciplinary approach.
• Strong communication and organizational skills.
• Ability to work independently and in collaboration with healthcare teams.
