Responsibilities:

• Collaborate with patients, families, physicians, and the healthcare team to develop, implement, and revise comprehensive case management plans.
• Lead coordination of patient care across disciplines, ensuring timely and appropriate interventions.
• Monitor patient clinical progress, diagnostic testing, treatments, and procedures to align with care plans and covered services.
• Consult with physicians regarding resource use, treatment plans, estimated length of stay, and discharge planning.
• Manage complex cases including frequent emergency department users, chronic pain patients, individuals with substance abuse issues, and those experiencing homelessness.
• Facilitate patient transfers to tertiary care centers, ensuring continuity of treatment and information accuracy.
• Collect and oversee data to meet regulatory and accreditation requirements.
• Conduct focused reviews on frequent emergency department visitors to improve care strategies.

Qualifications:

• Valid RN licensure with experience in home health nursing.
