Desired Qualifications and Experience:

• RN license, valid and in good standing
• Experience in Home Health preferred
• Strong assessment, case management, and critical thinking abilities
• Effective communication and collaboration skills
• Ability to work with complex and high-acuity patient populations

Role Responsibilities:

• Provide exceptional patient care, collaborating closely with families, physicians, and multidisciplinary teams
• Lead coordination and timely delivery of patient care across the care team
• Monitor care plans to ensure consistency with clinical needs and covered services
• Liaise with physicians regarding resource utilization, treatment plans, and discharge planning
• Address unique needs of complex patients including those with chronic pain, frequent ED visits, substance abuse, and homelessness
• Facilitate patient transfers and ensure seamless handoff of clinical information
• Collect and analyze data to maintain regulatory and accreditation standards
• Conduct focus reviews for frequent emergency department admissions
