Schedule/Hours

• 13-week contract, 40 guaranteed hours per week (except orientation/holidays)
• Day shifts: 10 hours (8:00–18:00)
• Shift options: Thurs–Sun or Sat–Tues rotation
• No call requirements; floating may occur based on census

Qualifications & Experience

• Active WI RN license and BLS (American Heart Association)
• Valid driver’s license required
• 1–2 years of current Home Health and Hospice experience (admissions, symptom management, death visits)
• Recent hands-on experience with Oasis documentation
• Epic charting proficiency (within 12 months)
• Strong assessment, time management, and adaptability skills
• Resilient self-starters comfortable with independent field work
• Ability to thrive in unpredictable environments (allergies/pets/smoke)
• Physical stamina to drive between visits and navigate home entrances/stairs
• Covid-19 vaccination required unless exempt for medical or religious reasons

Responsibilities

• Deliver 1:1 patient care for adults managing chronic illness (CHF, COPD, Diabetes, Cancer, CVA, wound care)
• Conduct home visits (7–7.5 per 10-hour day, 50–100 miles daily)
• Maintain timely, accurate documentation (OASIS/Epic)
• Adhere to home health, hospice, and infection prevention protocols
• Promote effective communication with patients, families, and multidisciplinary teams
