Hospice Registered Nurse - Home Health in Newark, NJ

Step into a highly rewarding clinical nursing position focused on home health and hospice care. This contract opportunity is ideal for an experienced RN passionate about delivering compassionate, patient-centered care to individuals and families during challenging times.

Schedule:

• 37.5 hours per week
• 7.5-hour shifts, 8:00 a.m.–4:00 p.m.
• Five days per week
• Every other weekend is required (with comp days provided during the week)
• Expect 4–5 patient visits per day, including admissions
• Flexibility to float to different counties as needed

Key Responsibilities:

• Conduct comprehensive patient assessments for hospice appropriateness and care planning
• Develop, implement, and revise individualized care plans using evidence-based practice
• Case management oversight for assigned patients, prioritizing care needs and effective interdisciplinary communication
• Document assessments and care via EMR; complete admissions and transfers within 24 hours
• Patient and family education using age-appropriate materials and documentation of teaching
• Routine medication reconciliation and safe medication management
• Uphold infection control and safety standards for patients and team members
• Attend regular interdisciplinary and staff meetings, participate in PI activities, and mentor new staff as needed

Qualifications:

• Minimum of two years of RN experience in home hospice
• Associate’s degree in nursing (ASN) or diploma with current RN license
• Active New Jersey RN license and driver’s license
• BLS certification required
• Strong EMR, critical thinking, and communication skills
• Reliable transportation for travel within assigned areas

Preferred:

• Three or more years of hospice RN experience

$2,001 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 36 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
