Registered Nurse - Home Health in Newton Center, MA

Seeking an experienced Registered Nurse to deliver exceptional patient care in a home health setting. This role requires a professional who can work independently, travel for patient visits, and take ownership of case management, ensuring high standards of community health practice.

Key Qualifications:

• Current, active RN license
• Minimum 1 year of certified home care nursing experience
• At least 1 year of proven case management experience
• Ability and willingness to travel to patient homes as assigned
• Proficient with electronic health records and interdisciplinary communication
• Strong assessment, planning, and patient education skills
• Committed to patient safety, confidentiality, and organizational best practices

Schedule:

• Weekend rotation required once every six weeks

Primary Responsibilities:

• Conduct comprehensive patient assessments in the home care environment
• Develop and implement individualized care plans to address patients’ specific health needs
• Provide information, instruction, and emotional support to patients and families to encourage wellbeing and engagement in care
• Execute admission processes for new cases in alignment with established standards and policies
• Maintain timely and accurate electronic documentation for all patient interactions
• Collaborate with physicians, therapists, and other interdisciplinary team members to coordinate care delivery
• Serve as a point of contact for case management, ensuring all patient care activities meet organizational guidelines
• Participate in quality improvement initiatives and adhere to community health best practices
