Registered Nurse - Home Health in Newton Center, MA

Schedule/Hours:

• Weekend rotations required (once every 6 weeks)
• Must be willing to work weekends, holidays, and participate in the on-call rotation per department needs

Qualifications:

• Active Registered Nurse (RN) license in Massachusetts
• Minimum 1 year of certified home care experience
• Minimum 1 year of case management background
• Reliable, independent transportation for home visits
• Commitment to weekend, holiday, and on-call responsibilities as scheduled

Key Responsibilities:

• Delivering clinical nursing care to patients from infancy to over 100 years of age in their homes
• Conducting thorough patient assessments and developing, implementing, and revising individualized care plans
• Providing education and support to patients and their families regarding health management, treatments, and preventive care
• Managing admissions and all aspects of patient case management in alignment with established healthcare standards and facility protocols
• Ensuring accurate and timely electronic documentation for all patient encounters
• Collaborating with a multidisciplinary team to coordinate comprehensive patient care
• Facilitating effective communication within the healthcare team and with patients’ families
• Maintaining compliance with all organizational policies and regulatory requirements
