Home Health RN in Newton Center, MA

Deliver a high standard of compassionate care within the comfort of patients' homes as part of a dedicated home health nursing team. This contract role requires a skilled Registered Nurse with home health experience and strong case management abilities, ready to support individuals across the age spectrum—from infants to seniors—through comprehensive assessments, personalized care planning, and interdisciplinary collaboration.

Schedule/Hours:

• Weekend rotations once every 6 weeks
• Must be available to work weekends, holidays, and on-call based on department rotation

Qualifications:

• Active RN license in Massachusetts
• Minimum of 1 year of certified home care nursing experience
• At least 1 year of case management experience
• Reliable, independent transportation for patient home visits
• Commitment to covering weekends, holidays, and on-call shifts as scheduled

Key Responsibilities:

• Deliver direct clinical nursing services to patients in their homes, covering all age groups
• Conduct thorough patient assessments and develop individualized care plans
• Provide education, resources, and support to patients and family members for better healthcare outcomes
• Perform admissions and maintain effective case management for all assigned patients
• Ensure timely, accurate electronic documentation in compliance with current healthcare standards and organizational policies
• Maintain consistent interdisciplinary communication to facilitate optimal patient care
