Hospice Registered Nurse - Home Health in Paterson, NJ

Experienced and compassionate registered nurses are sought to deliver exceptional home hospice care on a contract basis. In this role, you will support patients and families with comprehensive nursing assessments, education, and evidence-based care planning in the home setting.

Schedule:

• 5 days per week, 8:00 AM – 4:00 PM
• Every other weekend with compensated weekdays off
• Expect 4-5 home visits per day, including admissions
• Coverage may float to various counties as needed

Qualifications:

• Current RN license in New Jersey
• Valid New Jersey driver’s license
• BLS certification
• Associate’s degree in Nursing (ASN) or RN diploma
• Minimum 2 years experience as a registered nurse in a home hospice environment
• Proficiency in EMR documentation; strong critical thinking, communication, and organizational skills
• Ability to read, write, and communicate effectively in English

Preferred:

• 3+ years of RN experience in hospice or home health

Key Responsibilities:

• Conduct initial and ongoing patient assessments for hospice eligibility and plan of care development
• Educate patients and families using age-appropriate methods and materials
• Perform admissions, revisit documentation, and discharge assessments in accordance with timelines
• Ensure safe medication management and reconciliation per established guidelines
• Maintain interdisciplinary communication and attend care team meetings
• Supervise and support home health aides; participate in performance improvement activities and staff mentoring
• Adhere to infection control practices and uphold a safe environment for patients and colleagues

$2,001 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 36 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
