Registered Nurse - Home Health - Quincy, MA

Schedule/Hours:

• Weekend rotations required once every six weeks

Qualifications:

• Active RN license required
• Minimum of 1 year of certified home care nursing experience
• At least 1 year of case management experience
• Ability to travel to patient homes as assigned
• Proficiency with electronic documentation and interdisciplinary communication

Responsibilities:

• Assess patient conditions and develop tailored care plans aligned with organizational philosophy, policy, and standards of community health
• Provide hands-on nursing care and ongoing health education to patients and their families in the home setting
• Perform admissions processes, including comprehensive assessments and care planning
• Manage caseload effectively, ensuring continuity and quality of care through ongoing case management
• Maintain timely, thorough, and accurate electronic documentation for all patient encounters and communications
• Collaborate with a team of healthcare professionals to support patient goals and address care barriers
