Deliver high-quality, compassionate care directly to patients in their homes as an experienced Registered Nurse specializing in home health. This contract opportunity requires providing clinical services across all age groups—from infants to older adults—playing a pivotal role in improving quality of life for individuals in the community.
Schedule/Hours:
- Weekend rotations required (once every 6 weeks)
- Participation in holiday and on-call rotations per department needs
Qualifications:
- Minimum 1 year of certified home care nursing experience
- At least 1 year of case management experience
- Active Registered Nurse license in Massachusetts
- Reliable, independent transportation for home visits
- Flexibility to work weekends, holidays, and on-call shifts as scheduled
Responsibilities:
- Provide skilled nursing care to a diverse patient population, including assessments, care plan implementation, and education for patients and families
- Manage patient admissions and ongoing case management according to current healthcare standards and best practices
- Maintain accurate and timely electronic documentation to ensure seamless interdisciplinary communication and continuity of care
- Collaborate closely with other healthcare professionals to coordinate comprehensive care and support optimal patient outcomes
- Serve as a dependable resource for patients and families, delivering support and education throughout the care process
