This contract position requires a Registered Nurse (RN) to collaborate effectively with patients, families, physicians, and the multidisciplinary healthcare team. Responsibilities:
- Lead coordination of patient care with all disciplines within the care team, ensuring timely and appropriate interventions.
- Develop, implement, and revise individualized care plans consistent with patients’ clinical courses and covered services.
- Act as a consultant to the healthcare team regarding patient-specific needs and resource utilization.
- Discuss with physicians the suitability of treatment plans, consultations, length of stay, and discharge processes.
- Manage complex patient populations, including homeless individuals and those with chronic conditions.
- Facilitate transfers to tertiary care centers, ensuring thorough communication and continuity of care.
- Collect and oversee data for regulatory and accreditation purposes.
- Conduct focus studies on frequent emergency department users to optimize care strategies.
Qualifications:
- Active and valid RN license.
- Experience in home health nursing.
