A registered nurse with home health experience is sought for a contract position in Waipahu, HI. This role involves delivering exceptional patient care through thorough collaboration and case management oversight to optimize clinical outcomes.

Key responsibilities include coordinating patient care among the healthcare team, ensuring the interdisciplinary care plan aligns with patient needs and covered services, and serving as a consultant on specific patient issues. Additionally, this position requires oversight and data collection to maintain regulatory and accreditation compliance.

Qualifications/Desired Experience:

  • Active RN license with home health nursing experience
