Desired Qualifications:

• Active RN license valid in Hawaii or compact state eligibility
• At least one year of recent Home Health nursing experience
• Proficient in case management and interdisciplinary communication
• Strong organizational and critical thinking skills

Key Responsibilities:

• Collaborate in developing, implementing, revising, and reporting on the case management program
• Act as liaison among patients, families, nurses, physicians, and the interdisciplinary team
• Serve as a consultant on complex patient management and care coordination issues
• Ensure care plans are tailored to each individual’s unique needs, adjusting them as care evolves
• Monitor diagnostic tests, treatments, and procedures for appropriateness
• Communicate regularly with physicians to assess resource utilization and create comprehensive treatment and discharge plans
• Focus on high-acuity and complex cases, including patients with chronic conditions, frequent ED visits, and social challenges
• Facilitate smooth patient transfers and continuity of care, including necessary data sharing
• Gather and submit data for compliance with regulatory/accreditation requirements
• Conduct focus studies to proactively manage frequent emergency department visitors
