Qualifications:

  • Valid and active RN license
  • Demonstrated clinical experience in long term acute care (LTAC)
  • Proficient critical thinking and assessment skills
  • Current BLS certification required; ACLS preferred
  • Familiarity with electronic health records (EHR)
  • Strong communication and interpersonal abilities
  • Commitment to evidence-based, compassionate care

Schedule/Hours:

  • Shifts may vary based on facility needs
  • Flexibility to work evenings, nights, weekends, or holidays may be required

Responsibilities:

  • Deliver comprehensive nursing care to LTAC patients with acute, chronic, or complex diagnoses
  • Monitor vital signs, administer medications and treatments, and document observations accurately
  • Collaborate with an interdisciplinary healthcare team to develop and update patient care plans
  • Educate patients and families on procedures, interventions, and care processes
  • Respond promptly to changes in patient condition, performing emergency interventions as necessary
  • Maintain clear, precise patient documentation and follow all safety and infection control policies
  • Participate in ongoing professional development and facility training sessions
