Key Qualifications:

  • Active and unencumbered RN license
  • Minimum of 1 year of experience in Rehab, Long Term Care (LTC), or Long Term Acute Care (LTAC)
  • Strong clinical assessment, documentation, and communication skills
  • Proficiency in providing care to patients with complex medical needs
  • Ability to effectively collaborate with healthcare teams and support staff
  • Current BLS certification
  • Adaptability in fast-paced environments with quickly changing patient needs

Schedule/Hours:

  • Flexible scheduling options may be available, with contract duration discussed during the hiring process

Key Responsibilities:

  • Deliver direct, compassionate nursing care to patients in rehab and LTC settings
  • Develop individualized care plans and update them in response to patient progress
  • Monitor and document patient condition, reporting changes as needed
  • Administer medications and treatments according to physician orders and facility protocols
  • Support patients and families by providing education and emotional support
  • Ensure compliance with safety standards, infection control, and regulatory guidelines
  • Collaborate closely with physicians, therapists, and allied health professionals
