Qualifications & Experience:

Primary Responsibilities:

• Provide comprehensive nursing care to residents in a rehabilitation/long-term care unit
• Monitor, assess, and document patients’ health status and progress
• Administer medications and perform treatments in adherence to facility protocols
• Develop, implement, and update individualized care plans in collaboration with physicians and allied health professionals
• Educate residents and family members on treatment plans and health maintenance
• Ensure compliance with regulatory standards and facility policies
• Respond promptly to changes in residents’ conditions and coordinate appropriate interventions
