Desired Qualifications:

• Current, unencumbered RN license
• Recent experience in Rehab or Long-Term Care nursing
• Skilled in patient assessment, teaching, and development of care plans
• Knowledge of safe patient handling, infectious disease protocols, and safe work practices

Key Responsibilities:

• Perform comprehensive assessments within 8 hours of admission to form individualized care plans
• Identify patient education needs, involving families and interdisciplinary team members
• Provide timely and accurate documentation of care, interventions, and response
• Participate in and contribute to patient rehabilitation programs on assigned units
• Initiate discharge planning and patient teaching upon admission
• Communicate changes in patient status to physicians, charge nurses, and other healthcare team members
• Offer guidance and supervision to ancillary staff, ensuring safe, effective care delivery
• Maintain compliance with appearance, attendance, and safety standards
