Exciting opportunity for an experienced Registered Nurse with a strong background in Rehab or Long-Term Care. This contract assignment requires a dynamic and compassionate professional dedicated to delivering high-quality care while adapting to a rotating weekend schedule.

Schedule:

  • Rotating weekends
  • Varied shifts throughout Sunday to Saturday

Qualifications:

  • Valid RN license with state eligibility
  • Recent experience in rehabilitation and/or long-term care settings
  • Competence in assessment, documentation, and care planning for adult and geriatric populations
  • Strong communication skills to collaborate effectively with multidisciplinary teams, patients, and families
  • Ability to remain organized and focused during rotating schedules and changes in workload

Responsibilities:

  • Deliver skilled, patient-centered nursing care to rehab and long-term care residents, addressing physical, emotional, and social needs
  • Develop and update individualized care plans based on assessments and progress
  • Administer medications and treatments following best practices and facility policies
  • Monitor patient status, recognizing, and responding to clinical changes promptly
  • Educate patients and families on rehabilitation goals, health management, and discharge planning
  • Support a safe, respectful, and healing environment, proactively addressing concerns and contributing to quality improvement initiatives
  • Complete accurate documentation and report findings to the healthcare team in a timely manner
