Key Details:
- Schedule: Rotating weekends; shifts vary, Sunday through Saturday
Qualifications:
- Current, active RN license in good standing
- Previous experience in rehabilitation or long-term care settings preferred
- Solid clinical assessment, documentation, and care planning skills
- Strong communication and interpersonal abilities
- CPR/BLS certification (current and valid)
- Willingness to work flexible hours, including rotating weekends
Responsibilities:
- Assess, plan, implement, and evaluate individualized patient care
- Administer medications and treatments according to established protocols and physician orders
- Collaborate with interdisciplinary teams, patients, and families to facilitate safe transitions and optimal outcomes
- Maintain accurate medical records and documentation in compliance with facility standards
- Support patients and families by providing education on care plans, treatments, and rehabilitation processes
- Monitor patient progress and promptly report changes in condition
- Uphold infection control and safety standards throughout daily practice
