This rewarding contract opportunity calls for an experienced Registered Nurse (RN) specializing in Rehab/LTC to provide exceptional service and compassionate support to patients working toward recovery and wellness.

Schedule:

• Rotating weekends
• Shifts may be scheduled any day, Sunday through Saturday
• Hours vary based on facility needs

Qualifications:

• Current and unrestricted Registered Nurse (RN) license
• Previous experience in rehab or long-term care nursing strongly preferred
• Solid assessment, care planning, and documentation skills
• Strong communication, teamwork, and patient advocacy abilities
• Current BLS certification
• Adaptability to a fast-paced and supportive care setting

Responsibilities:

• Conduct regular patient assessments and develop individualized care plans in collaboration with interdisciplinary teams
• Monitor patient progress, report changes, and adjust care plans as needed
• Administer medications and treatments in accordance with physician orders and facility protocols
• Educate patients and families on care needs, rehabilitation goals, and discharge planning
• Foster a supportive and encouraging environment to promote patient independence and well-being
• Maintain clear, accurate, and up-to-date patient records
