Exciting opportunity available for a dedicated Registered Nurse with expertise in rehabilitation and long-term care settings. This contract position is ideal for professionals who are passionate about supporting residents in achieving optimal health outcomes within a collaborative care environment.
Key Qualifications:
- Active and unrestricted RN license
- Previous experience in a rehab or long-term care setting required
- Strong assessment, clinical, and documentation skills
- Ability to coordinate care with interdisciplinary teams
- Knowledge of resident safety protocols and infection control
- Compassionate approach to patient care and excellent communication abilities
Primary Responsibilities:
- Provide direct nursing care to residents, including medication administration, wound care, monitoring vital signs, and documenting patient progress
- Develop, implement, and regularly evaluate individualized care plans
- Collaborate with physicians, therapists, and other healthcare professionals to optimize patient outcomes
- Educate residents and their families on care plans, medications, and rehabilitative processes
- Respond promptly to changes in patient conditions and coordinate appropriate interventions
- Maintain compliance with all federal, state, and facility regulations related to long-term care and rehabilitation nursing
