Key Qualifications:

  • Active and unrestricted RN license
  • Previous experience in a rehab or long-term care setting required
  • Strong assessment, clinical, and documentation skills
  • Ability to coordinate care with interdisciplinary teams
  • Knowledge of resident safety protocols and infection control
  • Compassionate approach to patient care and excellent communication abilities

Primary Responsibilities:

  • Provide direct nursing care to residents, including medication administration, wound care, monitoring vital signs, and documenting patient progress
  • Develop, implement, and regularly evaluate individualized care plans
  • Collaborate with physicians, therapists, and other healthcare professionals to optimize patient outcomes
  • Educate residents and their families on care plans, medications, and rehabilitative processes
  • Respond promptly to changes in patient conditions and coordinate appropriate interventions
  • Maintain compliance with all federal, state, and facility regulations related to long-term care and rehabilitation nursing
