Deliver high-quality nursing care in a specialized rehabilitation and long-term care setting as a Registered Nurse (RN). This role involves supervising Licensed Practical Nurses, Certified Nursing Assistants, and new staff while ensuring adherence to individualized care plans. Responsibilities:
- Perform nursing assessments within 8 hours of patient admission to develop individualized care plans.
- Collaborate with patients, families, and interdisciplinary teams to identify educational needs and set care goals.
- Provide timely updates and documentation for team conferences and patient records.
- Participate actively in patient rehabilitation programs, delivering direct care and patient education.
- Initiate discharge planning from admission, preparing patients and families for transition.
- Communicate changes in patient status clearly among care providers to ensure continuity and safety.
- Supervise, direct, and train ancillary nursing personnel, delegating tasks effectively.
- Maintain patient and environmental safety, adhering strictly to hospital and nursing policies.
- Support a positive unit environment through calm leadership and team collaboration.
- Manage resources efficiently to promote quality outcomes and cost-effective care.
- Pursue ongoing professional development and participate in team-building activities.
- Assume charge nurse responsibilities as assigned after orientation.
Qualifications / Desired Experience:
- Current RN license with experience in Rehabilitation and Long-Term Care settings.
