Deliver exceptional patient care in a long-term care environment as a Registered Nurse specializing in Rehabilitation, Long-Term Acute Care, and Long-Term Care. This contract opportunity offers the chance to work in a supportive setting where your nursing expertise will directly impact patient recovery journeys and overall quality of life.

Key Responsibilities:

  • Provide care and support for patients requiring long-term, complex medical management
  • Assess and monitor patient health, administer medications, and communicate changes to the care team
  • Collaborate with physicians, therapists, and other healthcare professionals to develop and adjust individualized care plans
  • Document patient information accurately and maintain up-to-date medical records
  • Offer guidance and emotional support to patients and their families during the rehabilitation process
  • Ensure adherence to facility protocols and regulatory compliance for quality and safety
  • Participate in team meetings and contribute to quality improvement initiatives

Qualifications:

  • Current, valid RN license
  • Minimum of one year of recent experience in Rehab, LTAC, or Long-Term Care nursing
  • In-depth understanding of care plans, wound care, medication administration, and patient assessments
  • Strong critical thinking and problem-solving skills
  • Excellent communication skills and ability to work collaboratively with multidisciplinary teams
  • CPR/BLS certification (as required)
