Deliver exceptional patient care in a long-term care environment as a Registered Nurse specializing in Rehabilitation, Long-Term Acute Care, and Long-Term Care. Key Responsibilities:
- Provide care and support for patients requiring long-term, complex medical management
- Assess and monitor patient health, administer medications, and communicate changes to the care team
- Collaborate with physicians, therapists, and other healthcare professionals to develop and adjust individualized care plans
- Document patient information accurately and maintain up-to-date medical records
- Offer guidance and emotional support to patients and their families during the rehabilitation process
- Ensure adherence to facility protocols and regulatory compliance for quality and safety
- Participate in team meetings and contribute to quality improvement initiatives
Qualifications:
- Current, valid RN license
- Minimum of one year of recent experience in Rehab, LTAC, or Long-Term Care nursing
- In-depth understanding of care plans, wound care, medication administration, and patient assessments
- Strong critical thinking and problem-solving skills
- Excellent communication skills and ability to work collaboratively with multidisciplinary teams
- CPR/BLS certification (as required)
