A clinical pharmacist opportunity is available focusing on sterile compounding within a home infusion therapy setting. This contract position lasts approximately 13 weeks and offers a rotating schedule of 7:00 AM–3:30 PM one week, then 12:00 PM–8:00 PM the next.
Key Responsibilities:
- Evaluate patients for eligibility for home infusion therapy and develop individualized medication therapy plans based on clinical assessments, lab results, and pharmacokinetic data.
- Prepare, verify, and dispense sterile medications following USP standards and regulatory requirements.
- Educate patients, caregivers, and healthcare providers regarding medication use, infusion administration, and therapy management.
- Collaborate with healthcare teams to ensure continuity of care and monitor patient outcomes.
- Manage infusion pumps and related equipment while maintaining quality assurance and safety protocols.
- Provide drug information, maintain accurate documentation, and support compliance and medication safety programs.
Qualifications:
- Doctor of Pharmacy (PharmD) or graduate from an accredited School of Pharmacy.
- Active, unrestricted Texas Pharmacist license.
- One of the following: a minimum of one year sterile IV compounding experience; experience with non-sterile compounding, immunizations, and Medication Therapy Management (MTM); completion of an ASHP-accredited Pharmacy Residency; completion of an Advanced Pharmacy Practice Experience (APPE); or equivalent pharmacy experience.
- Strong clinical knowledge of home infusion pharmacy practices.
- Ability to interpret lab values and apply pharmacokinetic principles.
- Excellent communication and patient counseling skills.
This contract assignment is based in Arlington, TX. Candidates should be prepared to answer pre-screening questions regarding sterile compounding experience, certifications, and Texas licensure.
