Clinical Pharmacist in Auburn, NY

A direct hire opportunity is available for a Clinical Pharmacist specializing in infusion therapies. This role centers on preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications in a home infusion setting. Serving as a vital member of a multidisciplinary healthcare team, you will ensure the highest standards of sterile compounding, medication management, and patient education.

Core Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions within a sterile, ventilated environment in adherence to state and federal regulations.
• Monitor clinical parameters, assess lab values, and calculate pharmacokinetics for medications such as vancomycin and Total Parenteral Nutrition (TPN).
• Provide detailed patient counseling on specialty medication regimens and treatment plans.
• Educate patients and caregivers on the use of infusion pumps, sterile techniques, and catheter maintenance to support home care.
• Collaborate effectively with prescribing physicians, home health nurses, and internal teams—including intake, referral, reimbursement, nursing, and pharmacy operations—to ensure coordinated and efficient care delivery.
• Maintain compliance with all relevant regulatory requirements including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA standards.
• Oversee pharmacy inventory management, including temperature-sensitive products, and organize medication shipping for timely home delivery.

Qualifications:

• Minimum of 2 years' experience in infusion specialty pharmacy.
• Prior experience with Intravenous Immunoglobulin (IVIg) is a positive asset.
• Strong knowledge of specialty therapy payer requirements, reimbursement procedures, and contract compliance.
• Current and good standing New York State pharmacist license.
• Familiarity with the CareTend system for pharmacy management.
