Clinical Pharmacist in Baldwinsville, NY

An infusion clinical pharmacist plays a critical role in managing complex intravenous (IV) and subcutaneous medication therapies in a home infusion setting. This direct hire opportunity is based in Baldwinsville, NY, and offers a chance to be an essential part of a multidisciplinary healthcare team focused on exceptional patient care.

Responsibilities:

• Prepare specialty pharmaceuticals and sterile IV solutions in compliance with state and federal regulations.
• Monitor clinical parameters by assessing lab values and calculating pharmacokinetic dosing (e.g., vancomycin). Adjust Total Parenteral Nutrition (TPN) regimens as needed.
• Provide thorough patient and caregiver education on specialty medications and home infusion techniques, including infusion pump use and catheter maintenance.
• Coordinate care with prescribing physicians, home health nurses, and internal teams such as intake, referral services, reimbursement, nursing, clinical services, and pharmacy operations to ensure continuity and quality of care.
• Maintain regulatory compliance with State Board of Pharmacy regulations, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other applicable standards.
• Oversee pharmacy inventory management, including refrigerated items, ensuring accurate packing and timely shipment of medications and supplies to patients’ homes.

Qualifications:

• Minimum of 2 years’ experience in infusion specialty pharmacy.
• Experience with IVIg therapies is highly desirable.
• Strong understanding of specialty therapy payer requirements, reimbursement processes, and contract obligations.
• Current and active NY pharmacist license in good standing.
• Familiarity with CareTend system preferred.
