Clinical Pharmacist - Basalt, CO

An opportunity is available for an experienced Clinical Pharmacist proficient in Epic to support a hospital transitioning from Meditech to Epic EMR systems. This contract role begins as soon as possible and is projected to run through mid-August 2026, with flexibility in assignment length based on your start date. Located in Basalt, CO, this position involves a rotating shift schedule, working 40 hours per week including some weekends.

Schedule:

• Rotating shifts starting at 6:30 AM or 8:00 AM, Monday through Friday
• Rotating weekends from 7:00 AM to 12:00 PM

Primary Responsibilities:

• Prepare and dispense medication orders accurately according to physician requests
• Maintain medication stock in patient care areas ensuring availability and safety
• Monitor medication therapies for safety, effectiveness, and cost-efficiency
• Transcribe and maintain computerized patient medication profiles, including IV admixtures and chemotherapy products
• Ensure compliance with all federal, state, and local laws for controlled substances
• Monitor drug therapy for interactions, allergies, and dosing appropriateness
• Provide clinical consultations and educate medical staff, patients, and families as needed
• Participate in quality improvement and medication review activities
• Serve as a member of the hospital’s Code Response Team

Qualifications:

• Active Colorado Pharmacist License in good standing
• Graduation from an ACPE-accredited School of Pharmacy with a BS Pharmacy degree
• Experience with Epic EMR required; Meditech experience a plus
• Current BLS and ACLS certifications (American Heart Association)
• Flu vaccine required (no exemptions) from October 1 through April 1
