A clinical pharmacist opportunity is available at a healthcare facility in Basalt, CO, offering a vital role in medication management and patient care. This contract position requires an active Colorado Pharmacy license and expertise with Epic electronic medical records, as the facility is transitioning from Meditech to Epic. The assignment, starting ASAP and extending to mid-August 2026 or longer, offers a dynamic rotating schedule with 40 hours per week, including weekday shifts beginning at 6:30 AM or 8:00 AM and rotating weekend coverage from 7:00 AM to 12:00 PM.

Responsibilities:

  • Interpret and accurately transcribe physician medication orders into computerized patient medication profiles.
  • Prepare, compound, and dispense pharmaceuticals, including IV admixtures, chemotherapy, and parenteral nutrition products.
  • Monitor drug therapy regimens, identifying contraindications, interactions, allergies, and dosing appropriateness.
  • Ensure compliance with all federal, state, and local regulations, particularly regarding controlled substances.
  • Review culture and sensitivity data to recommend appropriate antibiotic use.
  • Provide clinical consultations and suggest cost-effective therapeutic alternatives.
  • Maintain formulary integrity through therapeutic substitutions and rational drug therapy selection.
  • Document clinical activities and participate in quality improvement initiatives.
  • Participate in staff education, training, and code response teams.

Qualifications:

  • Active Colorado Pharmacy license in good standing.
  • Graduate of an ACPE accredited School of Pharmacy with a BS Pharmacy degree.
  • BLS and ACLS certifications through the American Heart Association.
  • Proven experience with Epic electronic medical records is mandatory; experience with Meditech is a plus.
  • Flu vaccine is required between October 1 and April 1 with no exemptions.

Schedule:

  • Rotating day/evening shifts, 40 hours per week.
  • Start times at 6:30 AM or 8:00 AM, Monday through Friday.
  • Rotating weekends working 7:00 AM to 12:00 PM.
