Clinical Pharmacist | Basalt, Colorado

A clinical pharmacist role is available in Basalt, CO, offering a contract position designed to support medication management during a critical EMR transition. The assignment begins immediately and is projected through mid-August 2026, with flexibility to adjust the end date based on your start.

Work Schedule:

• Rotating shifts, 40 hours per week
• Start times at 6:30 AM or 8:00 AM, Monday through Friday
• Rotating weekend shifts from 7:00 AM to 12:00 PM

Core Responsibilities:

• Prepare and dispense medication orders in alignment with physician directives
• Maintain medication inventory in patient care areas
• Ensure safe, cost-effective medication therapy following protocols and regulations
• Review medication orders and maintain computerized patient medication records
• Compound and dispense pharmaceuticals, including IV admixtures and chemotherapy preparations
• Monitor drug therapies for safety and effectiveness, perform pharmacokinetic dosing
• Provide clinical consultation, drug information, and documentation to healthcare teams
• Participate in quality improvement initiatives and serve on the Code Response Team

Required Qualifications:

• Active Colorado Pharmacist License in good standing
• Graduate of an ACPE-accredited School of Pharmacy with a Bachelor of Science degree
• Current BLS (American Heart Association) certification
• Current ACLS (American Heart Association) certification
• Flu vaccination required between October 1 and April 1 (no exemptions)
• Must have EPIC EMR experience; Meditech training is a plus
