Clinical Pharmacist in Bellaire, TX

A dynamic opportunity is available for an experienced Clinical Pharmacist specializing in oncology to support a large, busy 500+ bed hospital located in the Texas Medical Center. This role focuses on providing expert pharmacy care as an Oncology Inpatient Pharmacist, working closely with the patient care team in a high-acuity environment.

Responsibilities include:

• Rounding with multidisciplinary patient care teams to offer clinical pharmacy interventions.
• Recommending and implementing medication therapies tailored to oncology patients.
• Developing and maintaining treatment protocols within Epic and Epic Beacon systems.
• Supporting smooth transitions of care by coordinating with outpatient oncology pharmacists.
• Managing non-formulary medication requests to ensure timely patient access.
• Coordinating specialty oncology medications to optimize patient outcomes.

Schedule:

• Primarily day shifts to support consistent, collaborative care.

Qualifications and Desired Experience:

• Current Texas Pharmacy License and Texas Intravenous Certification.
• Minimum of 3 years inpatient pharmacy experience, preferably in oncology.
• Proficiency with Epic Electronic Health Record system and Epic Beacon modules.
• Strong clinical judgment and ability to provide evidence-based medication recommendations.
