This opportunity is for a Clinical Pharmacist based in the State College, PA area, working in a hybrid role that blends clinical and staffing duties within a respected medical center. Approximately 60% of your time will involve clinical responsibilities focused on Population Health and Ambulatory Care, especially managing medication regimens for chronic conditions like diabetes. The remaining 40% will involve staffing duties, with the potential for this role to evolve into a fully clinical position as the program expands.
Responsibilities:
- Manage patient medication therapies within target disease states, with an emphasis on diabetes care
- Collaborate closely with healthcare teams to optimize medication outcomes in ambulatory care settings
- Participate in staffing duties, ensuring smooth pharmacy operations during shifts
- Contribute to the growth and development of clinical pharmacy services within the health system
Qualifications and Desired Experience:
- Active PA pharmacist license
- Experience in ambulatory care or inpatient pharmacy settings preferred
- Completion of a residency program is a strong plus
- Familiarity with Meditech electronic health record system is beneficial
- Strong passion for patient care and enthusiasm for developing clinical pharmacy programs
Schedule and Location:
- Variable days and afternoons, primarily day shifts
- 8-hour shifts
- Work is onsite at a medical center in State College, PA, offering a blend of clinical interaction and staffing duties
