Key Responsibilities:
- Review and verify medication orders to ensure accuracy, appropriateness, and adherence to regulatory standards.
- Provide clinical oversight for infusion therapies, including specialty medications, biologics, antimicrobials, TPN, and compounded sterile preparations.
- Evaluate patient-specific therapy plans and monitor clinical outcomes to optimize treatment effectiveness.
- Collaborate closely with physicians, nurses, and multidisciplinary teams to guide medication management and therapeutic decisions.
- Maintain compliance with all applicable state and federal pharmacy regulations, accreditation standards, and organizational policies.
Desired Qualifications:
- Licensed Pharmacist with a clear and active Washington State license.
- Minimum of 2 years clinical experience, preferably with direct involvement in home infusion pharmacy.
- Demonstrated clinical expertise in inpatient, IV, home infusion, TPN, vancomycin, biologics, and compounding procedures.
- Experience with specialty patient populations, including rare disease states and complex cases, is highly valued.
- Residency in the Seattle area or willingness to relocate locally.
