Per Diem Clinical Pharmacist in Big Bear City, CA

A healthcare facility in Big Bear City, CA, is seeking a Clinical Pharmacist to provide essential pharmacy services on a per diem basis. This role involves ensuring the safe and accurate use of medications in an inpatient setting, with responsibilities spanning medication verification, sterile compounding, and clinical consultation.

Schedule:

• Monday to Friday, Day Shift: 9:00 AM – 5:00 PM (Full-time)
• On-call availability for weekends and after-hours (calls are rare)

Key Responsibilities:

• Verify and review medication orders to ensure accuracy, appropriateness, and patient safety
• Compound sterile medications as needed, including IV admixtures and unit dose medications (no chemotherapy or hazardous drugs at this site)
• Respond promptly to emergency room codes, supporting acute patient needs
• Maintain regulatory compliance with hospital policies, state and federal laws, and sterile compounding standards
• Monitor patients’ medication therapies and provide clinical recommendations to physicians and nurses
• Refill Pyxis machines at two local family health centers as required

Qualifications / Desired Experience:

• Active and clear California Pharmacist license (required)
• Minimum of 2 years inpatient pharmacy experience (required)
• Experience with pharmacy software systems such as CSPI, Evident, or Pyxis is preferred; training available if needed
• Prior experience working independently as the sole pharmacist in a clinical setting is preferred
• Ability and willingness to fulfill all on-call hours (required)
