A Clinical Pharmacist with infusion expertise is needed for a direct hire opportunity in Camillus, NY. This role focuses on preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications in a home infusion setting. Serving as the central figure in a multidisciplinary team, you will oversee sterile compounding, adjust medication doses, and provide essential education to patients and caregivers.

Core Responsibilities:

  • Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment adhering to state and federal guidelines.
  • Assess laboratory values, calculate pharmacokinetic dosing (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) to optimize patient treatments.
  • Provide thorough counseling and education regarding specialty medications and treatment plans.
  • Teach patients and caregivers proper use of infusion pumps, sterile technique, and catheter maintenance at home.
  • Collaborate with prescribing physicians, home health nurses, and internal teams (intake, referral, reimbursement, clinical services, pharmacy operations) to ensure coordinated care and prevent hospital readmissions.
  • Maintain regulatory compliance with State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other standards.
  • Manage pharmacy inventory including refrigerated items, and prepare medications and supplies for timely and safe delivery.

Qualifications:

  • Minimum of 2 years’ experience in infusion specialty pharmacy.
  • Experience with IV immunoglobulin (IVIg) is advantageous.
  • Familiarity with specialty therapy payer requirements, reimbursement, and contract obligations.
  • Current and active NY pharmacist license in good standing.
  • Experience using the CareTend system preferred.
