A home infusion clinical pharmacist plays a vital role in preparing, dispensing, and monitoring complex intravenous (IV) and subcutaneous medications, ensuring patients receive expert care in the home setting. This position functions as the central hub of a multidisciplinary team, overseeing sterile compounding, adjusting medication doses, and providing critical education to patients and caregivers.

Key Responsibilities:

• Sterile Compounding: Accurately prepare specialty pharmaceuticals and IV solutions in a controlled sterile environment in compliance with state and federal regulations.
• Clinical Monitoring: Evaluate lab values, perform pharmacokinetic dosing calculations (such as vancomycin), monitor Total Parenteral Nutrition (TPN), and provide detailed patient counseling on specialty medication regimens.
• Patient Education: Train patients and caregivers on infusion pump operation, sterile techniques, and catheter maintenance to support safe home infusion therapy.
• Care Coordination: Work collaboratively with prescribing physicians, home health nurses, intake and referral teams, and pharmacy operations to ensure coordinated therapy management and prevent hospital readmissions.
• Regulatory Compliance: Adhere to all relevant regulations including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA standards.
• Inventory and Shipping: Manage pharmacy inventory, including refrigerated items, and ensure medications and supplies are safely packaged and shipped for timely delivery.

Qualifications/Desired Experience:

• Minimum of 2 years’ experience in infusion specialty pharmacy
• IVIg experience is highly desirable
• Strong knowledge of specialty therapy payer requirements, reimbursement processes, and contract obligations
• Current and valid NY pharmacist license in good standing
• Experience with CareTend system preferred
