Clinical Pharmacist - Carbondale, Colorado

A clinical pharmacy role is available on a contract basis, starting immediately, with an assignment projected through mid-August 2026. This position involves working rotating shifts, 40 hours per week, with weekday start times at 6:30 AM or 8:00 AM, and rotating weekends from 7:00 AM to 12:00 PM. The role supports a facility transitioning its electronic medical records from Meditech to Epic, requiring strong Epic system experience. Meditech training is considered a plus.

Responsibilities include preparing and dispensing medications as prescribed by physicians, maintaining medication inventories, and ensuring safe and cost-effective drug therapies in accordance with applicable laws and institutional protocols. Key duties encompass interpreting medication orders, compounding pharmaceuticals including IV admixtures and chemotherapy, monitoring drug therapy for interactions and contraindications, performing pharmacokinetic dosing, reviewing culture and sensitivity data, and providing clinical consultation to medical staff. The pharmacist will also participate in quality improvement initiatives, document clinical interventions, deliver education and training to healthcare teams, and serve as a member of the Code Response Team.

Qualifications:

• Active pharmacy license in Colorado in good standing
• Graduation from an ACPE-accredited School of Pharmacy with a Bachelor’s degree
• Current American Heart Association BLS and ACLS certification
• Demonstrated proficiency with Epic; Meditech experience helpful
• Flu vaccination compliance (no exemptions permitted) between October 1 and April 1
