Clinical Pharmacist - Carbondale, Colorado

A pharmacist opportunity is available for a clinical role supporting a healthcare facility transitioning from Meditech to Epic EMR systems. This contract position offers a rotating shift schedule and involves working 40 hours per week with weekend rotations. The assignment is expected to last until mid-August 2026, with flexibility in start dates.

Location:

• Carbondale, Colorado

Schedule:

• Rotating day/evening shifts starting at 6:30 AM or 8:00 AM Monday through Friday
• Rotating weekends from 7:00 AM to 12:00 PM

Key Responsibilities:

• Prepare and dispense medication orders accurately according to physician prescriptions.
• Maintain medication inventory in patient care areas and ensure compliance with federal, state, and local regulations.
• Monitor and review drug therapy regimens for interactions, contraindications, allergies, and dosing appropriateness.
• Perform pharmacokinetic dosing and interpret culture & sensitivity data to recommend appropriate antibiotic therapies.
• Provide clinical consultations, recommend cost-effective therapeutic alternatives, and offer drug information to patients and healthcare teams.
• Document clinical activities and participate in quality improvement and medication review initiatives.
• Participate in training programs for pharmacy, nursing, and medical staff, and serve as a member of the Code Response Team.

Qualifications:

• Active Colorado Pharmacist License in good standing.
• Graduate of an ACPE-accredited School of Pharmacy with a Bachelor of Science in Pharmacy.
• Experience with Epic EMR system is mandatory; Meditech training is a plus.
• Current American Heart Association BLS and ACLS certifications.
• Required flu vaccination during flu season (October 1 through April 1).
