An opportunity is available for a Per Diem Clinical Pharmacist specializing in Home Infusion to provide essential pharmaceutical care and support in a home infusion environment. This role emphasizes accuracy, communication, and patient care through direct interaction with medical offices, nursing staff, and patients to ensure safe and effective medication therapies.

Schedule:

  • Monday to Friday, 9:00 AM – 5:00 PM
  • Coverage periods include: 4/14–4/22, 6/13–6/25, and 7/29–8/5

Key Responsibilities:

  • Verify pharmaceutical products and medications entering the clean room for sterile compounding based on batch records and lot numbers
  • Conduct final checks on compounded sterile products (CSPs) including labels, fill amounts, and drugs used prior to release
  • Inspect outgoing shipments ensuring accuracy of medications and medical supplies as per delivery tickets
  • Communicate effectively with physicians, field nurses, and patients regarding drug regimens, inventory status, adverse drug reactions, and abnormal lab results
  • Follow up with patients regarding upcoming deliveries, inventory levels, and medication adherence; escalate any concerns to management
  • Handle phone inquiries from medical offices, nursing agencies, and patients professionally and promptly

Qualifications and Experience:

  • Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) or Master’s degree in Pharmacy preferred
  • Current pharmacist license in North Carolina
  • Minimum two years recent experience in acute-care pharmacy or one year in home infusion therapy
  • Strong expertise in intravenous admixture services and intravenous therapy
  • Proficiency with CPR+ software is mandatory
