Per Diem Clinical Pharmacist - Ceres, California

A healthcare organization is seeking a dedicated clinical pharmacist for a per diem role supporting their infusion pharmacy services in Ceres, CA. This position offers a regular part-time schedule of 2-3 days per week, Monday through Friday, from 8:30 a.m. to 5:00 p.m., ideal for candidates residing in nearby areas such as Modesto, Stockton, or Merced. Start date is immediate with ongoing assignment duration.

Key Responsibilities:

• Review physician and prescriber orders to verify dosage accuracy, drug selection, administration routes, and patient suitability based on diagnosis and medication interaction.
• Maintain comprehensive patient records including height, weight, medical history, laboratory results, and current medication regimens (prescription and over-the-counter).
• Support regulatory compliance by managing documentation required by state, federal, and accrediting bodies, including JCAHO and ACHC.
• Coordinate and interpret clinical laboratory tests to ensure patient safety and therapeutic effectiveness, collaborating with nurses and physicians.
• Provide clinical and technical assistance to healthcare team members and patients.
• Assist the pharmacist-in-charge with policy implementation and supervise pharmacy staff and trainees.
• Oversee sterile compounding procedures and quality assurance programs, including validation and environmental testing.
• Document communications with healthcare professionals and patients accurately in medical records.
• Promote safe pharmacy practices and deliver patient education regarding drug therapies and administration.

Qualifications:

• Active California pharmacist license in good standing.
• Proven expertise in IV, infusion, and clinical pharmacy, with preference for prior home infusion pharmacy experience.
• Familiarity with CareTend pharmacy management system is advantageous.
