A leading infusion pharmacy is seeking a part-time clinical pharmacist to provide expert support at their Ceres, CA location. This role offers 2-3 days per week of weekday shifts, Monday through Friday, from 8:30 a.m. to 5:00 p.m., making it ideal for candidates based in Modesto, Stockton, or Merced. The position starts immediately and will continue on an ongoing basis.

Key Responsibilities:

  • Review and evaluate physician prescription orders for dosage accuracy, drug choice suitability, potential interactions, administration routes, and medication stability.
  • Maintain comprehensive patient records, including medical history, lab results, current medications (prescription, OTC, herbal, nutritional), height, and weight.
  • Ensure compliance with documentation requirements set by state and federal agencies and accrediting organizations such as JCAHO and ACHC.
  • Order and assess clinical laboratory tests, collaborating with nurses and physicians to optimize patient medication and nutritional therapies.
  • Provide clinical and technical support to the healthcare team, including physicians, nurses, and patients.
  • Assist the pharmacist-in-charge with implementing new policies, supervising pharmacy staff, and maintaining quality assurance programs for sterile compounding.
  • Document all professional communications in patient medical records and promote safety by following excellent pharmacy practices.
  • Educate patients and caregivers on drug therapies as part of comprehensive care.

Qualifications:

  • Valid California pharmacist license in good standing.
  • Strong background in IV, infusion, and clinical pharmacy practice.
  • Prior experience in home infusion pharmacy is highly preferred.
  • Familiarity with CareTend pharmacy system is a plus.
