Per Diem Clinical Pharmacist in Chicora, PA

A part-time Clinical Pharmacist position is available to provide skilled pharmaceutical care primarily in long-term care settings. This role requires 24 hours per week and is an ongoing assignment starting with a one-month duration, with the potential for extension.

Location: Chicora, PA
Schedule: 24 hours per week, Monday through Friday, 8:00 AM – 6:00 PM

Key Responsibilities:

• Review medication orders entered by data entry technicians for skilled nursing facilities, personal care facilities, and hospice patients
• Verify packaged medications, including blister packs, sterile, and non-sterile medications
• Respond to phone inquiries from nursing facilities, hospice nurses, and physicians, providing clinical guidance
• Perform controlled substance counts and maintain detailed perpetual inventory documentation
• Inspect emergency medication boxes to ensure compliance and preparedness

Qualifications:

• Bachelor’s degree in Pharmacy
• Active pharmacist license in Pennsylvania, in good standing
• A minimum of 1 year of pharmacist experience, with preference for long-term care (LTC) exposure
• Ability to sit, stand, and perform tasks requiring fine motor skills and visual acuity
• Capable of lifting up to 30 pounds as required

Working with systems such as QS1, Docutrack, and PCC is essential for this role, supporting effective medication management and documentation.
