Role Overview
This role spans clinical and operational pharmacy responsibilities, giving you a genuinely well-rounded day rather than a purely repetitive dispensing role. Approximately 70 percent of your time will be focused on clinical PV1 duties, 25 percent on operational PV2 verification and dispensing, and 5 percent on regulatory compliance. This role keeps you clinically sharp while giving you the operational grounding that makes a great long-term care pharmacist.
Clinical Responsibilities, PV1
-Review, receive, and verify all new patient orders and document accordingly
-Conduct initial and ongoing assessment of patient, drug, and disease information to identify real and potential drug-related problems
-Assist physicians and nursing staff in setting therapeutic objectives and establishing monitoring parameters for drug therapy safety and efficacy
-Monitor drug therapy outcomes by collecting, evaluating, and interpreting laboratory data, clinical data, and other pertinent patient information
-Communicate therapeutic drug monitoring results to physicians, nursing and other appropriate team members
-Provide consultation and answer inquiries from patients, families, physicians, and other healthcare professionals
-Identify and implement appropriate formulary substitutions
-Match appropriate drug therapy with diagnosis to ensure efficacy
-Maintain thorough documentation of all clinical activities and communications in the patient record
-Stay current on pharmacotherapy through literature review and continuing education
Operational Responsibilities, PV2
-Process and check all medications, supplies, and equipment before dispensing from the pharmacy
-Maintain pharmacy records in accordance with applicable laws and regulations.
-Participate in weekend, holiday, and on-call rotation as scheduled
-Assist with orientation and training programs for new and current staff
-Attend designated department meetings as scheduled
Regulatory Responsibilities
-Dispense all pharmaceutical and related supplies and equipment in full compliance with federal, state, and local laws and standards of practice governing pharmacy.y
Experience
-Minimum 1 to 3 years long-term care pharmacy experience required
-PV1 and PV2 verification experience required
-Frameworks pharmacy system experience highly preferred but not required
-Comfortable with therapeutic drug monitoring and clinical documentation in an LTC setting
Skills
-Strong clinical judgment across a diverse long-term care patient population
-Detail-oriented with excellent accuracy in order verification and dispensing.
-Effective communicator with physicians, nursing staff, and healthcare team members
-Reliable and consistent across a rotating weekend schedule
-Comfortable managing minimal on-call responsibilities
