An opportunity is available for a Clinical/Infusion Pharmacist to join a healthcare team in Columbus, OH. This direct-hire role involves managing and supporting home infusion therapy patients, preparing sterile compounded medications, and working closely with multidisciplinary professionals to ensure optimal patient outcomes.

Schedule and Location:

  • Monday through Friday, 8:00 AM to 4:30 PM
  • Participate in an on-call rotation
  • Columbus, OH

Key Responsibilities:

  • Prepare specialty pharmaceuticals and intravenous (IV) solutions in sterile environments, adhering to all state and federal regulations
  • Monitor patient clinical status by assessing lab values and calculating pharmacokinetic dosing for medications such as vancomycin
  • Oversee Total Parenteral Nutrition (TPN) management and adjust therapies as necessary
  • Provide thorough education and counseling to patients and caregivers on infusion pump use, sterile technique, and catheter care at home
  • Collaborate with physicians, home health nurses, and internal teams to coordinate care and prevent readmissions
  • Maintain compliance with regulatory standards from State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and others
  • Manage pharmacy inventory, including refrigerated medications, and prepare shipments to patients’ homes

Qualifications and Experience:

  • Minimum of two years in infusion specialty pharmacy practice
  • IVIg (Intravenous Immunoglobulin) experience is a plus
  • Familiarity with specialty therapy payer requirements, reimbursement processes, and contractual obligations
  • Proficient with CareTend system
  • Current, valid Ohio pharmacist license in good standing
