Clinical Pharmacist - Columbus, Ohio

A leading Long Term Care pharmacy in Columbus, OH, is seeking two experienced IV Pharmacists to join their expanding team. This opportunity offers direct hire positions with flexible weekday schedules across day and evening shifts.

Open Positions:

• IV Lead Pharmacist: 1st shift, Monday to Friday, flexible hours between 7:00 AM – 4:30 PM
• IV Staff Pharmacist: 2nd shift, Monday to Friday, flexible hours between 4:30 PM – 2:30 AM

Candidates must hold an active Ohio pharmacist license.

Key Responsibilities:

• Prepare, check, and dispense intravenous medications, including hazardous drugs, TPN, and biologics, adhering to USP standards and protocols.
• Manage clinical therapy by evaluating drug orders, monitoring for interactions, dosage appropriateness, and patient response, particularly in complex disease states such as oncology, immunology, or transplant.
• Educate patients on medication administration, therapy expectations, and side effect management.
• Maintain compliance with USP 797/800, manufacturer guidelines, state and federal regulations, and ensure cleanroom quality standards.
• Collaborate effectively with healthcare professionals, including nurses and physicians, to coordinate patient care and medication delivery.
• Manage inventory and prior authorizations to support uninterrupted patient therapy.

Desired Qualifications:

• Valid pharmacist license in Ohio.
• Experience in sterile compounding and preparation of IV and biologic medications.
• Strong clinical knowledge of complex high-cost medication therapies.
• Familiarity with regulatory compliance in sterile compounding environments.
• Effective communication skills for interdisciplinary collaboration and patient education.

Schedule:

• IV Lead Pharmacist: 1st Shift (7:00 AM – 3:30 PM or 8:00 AM – 4:30 PM), Monday – Friday
• IV Staff Pharmacist: 2nd Shift (4:30 PM – 12:00 AM or 6:00 PM – 2:30 AM), Monday – Friday
