An opportunity is available for a home infusion clinical pharmacist dedicated to optimizing complex intravenous (IV) and subcutaneous medication therapies. This direct hire role in Columbus, OH, is ideal for a clinical pharmacist who thrives in a multidisciplinary team environment and is committed to enhancing patient outcomes through expert medication management and education.

Key Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions in compliance with sterile compounding standards and regulatory guidelines.
• Assess lab values and calculate pharmacokinetic dosing, including vancomycin and Total Parenteral Nutrition (TPN), to ensure safe, effective treatments.
• Provide patient and caregiver education on infusion pump use, sterile technique, and catheter care to promote safe home therapy.
• Collaborate with physicians, home health nurses, and internal teams—such as referral services, reimbursement, nursing, and pharmacy operations—to coordinate care and reduce hospital readmissions.
• Maintain compliance with all relevant regulations, including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
• Oversee pharmacy inventory, including refrigerated medications, and ensure timely, secure shipping to patients' homes.

Desired Experience and Qualifications:

• Minimum of 2 years’ experience in infusion specialty pharmacy.
• Experience with IVIg therapies is advantageous.
• Strong understanding of specialty therapy payer requirements, reimbursement, and contract obligations.
• Current, valid NY pharmacist license in good standing.
• Proficiency with CareTend pharmacy system.
