A Clinical Pharmacist specializing in home infusion services is needed to prepare, dispense, and monitor complex intravenous (IV) and subcutaneous medications for patients. This role serves as a vital link within a multidisciplinary healthcare team, ensuring high-quality, compliant, and patient-centered pharmaceutical care in a home setting.

Key Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions using sterile compounding techniques in accordance with all relevant state and federal regulations.
• Assess lab values, calculate pharmacokinetic dosing (e.g., vancomycin), monitor Total Parenteral Nutrition (TPN), and adjust medication regimens as necessary.
• Provide thorough patient and caregiver education on infusion pumps, sterile techniques, and catheter care to ensure safe home administration.
• Collaborate with physicians, home health nurses, and internal teams including intake, referral services, reimbursement, clinical services, and pharmacy operations to optimize therapy outcomes and reduce hospital readmissions.
• Ensure ongoing compliance with regulatory requirements from organizations such as State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
• Manage pharmacy inventory including refrigerated medications and oversee the packing and shipping of supplies to patients’ homes in a timely manner.

Qualifications / Desired Experience:

• Minimum 2 years of experience in infusion specialty pharmacy.
• Experience with Intravenous Immunoglobulin (IVIg) preferred.
• Knowledge of specialty therapy payer requirements, reimbursement procedures, and contract obligations.
• Current and active Ohio pharmacist license in good standing.
• Proficient with CareTend system or similar pharmacy management software.
