A home infusion clinical pharmacist plays a crucial role in managing the preparation, dispensing, and monitoring of complex intravenous (IV) and subcutaneous medications for patients receiving care at home. This position demands clinical expertise and collaboration with a multidisciplinary team to ensure safe and effective therapy while providing essential education and support to patients and caregivers.
Responsibilities:
- Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment, adhering strictly to state and federal regulations.
- Assess lab values, perform pharmacokinetic dosing calculations (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) to adjust treatment plans accordingly.
- Provide comprehensive patient counseling and education regarding specialty medications, infusion pump use, sterile techniques, and catheter maintenance.
- Collaborate with prescribing physicians, home health nurses, and internal teams—including intake, referral, reimbursement, nursing, clinical services, and pharmacy operations—to ensure seamless therapy management and prevent hospital readmissions.
- Maintain compliance with regulatory requirements, including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, OSHA, and other relevant standards.
- Oversee pharmacy inventory management, including refrigerated products, and coordinate packing and shipping of medications to patients' homes.
Qualifications and Desired Experience:
- Current and good standing Ohio pharmacist license.
- Minimum 2 years of experience in infusion specialty pharmacy.
- Experience with IVIg therapies is a plus.
- Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations.
- Proficiency with CareTend system preferred.
