Home Infusion Clinical Pharmacist | Crestwood, Kentucky

A direct hire opportunity is available for a Clinical Pharmacist specializing in home infusion therapy in the greater Louisville, KY area. The pharmacist will prepare, dispense, and monitor complex intravenous (IV) and subcutaneous medications, playing a pivotal role on a multidisciplinary healthcare team. This position offers a Monday to Friday schedule, 8:30 a.m. to 5:00 p.m., with an immediate start.

Core Responsibilities:

• Prepare specialty pharmaceuticals and IV solutions in a sterile, ventilated environment adhering to state and federal regulations.
• Assess laboratory values, calculate pharmacokinetic dosing (e.g., vancomycin), and monitor Total Parenteral Nutrition (TPN) to optimize patient treatment.
• Provide thorough education and counseling to patients and caregivers about specialty medications and infusion therapy.
• Educate patients and caregivers on infusion pump usage, sterile techniques, and catheter care for home administration.
• Collaborate with prescribing physicians, home health nurses, and internal departments to coordinate therapy and minimize hospital readmissions.
• Ensure compliance with regulatory standards including State Board of Pharmacy, HIPAA, FDA, DEA, Medicare, ACHC, URAC, and OSHA.
• Manage pharmacy inventory, including refrigerated products, and prepare medications and supplies for safe, timely home delivery.

Qualifications and Desired Experience:

• At least 2 years of infusion specialty pharmacy experience.
• IVIg experience is a plus.
• Familiarity with specialty therapy payer requirements, reimbursement processes, and contract obligations.
• Current and active Kentucky pharmacist license in good standing.
• Experience with CareTend system preferred.
